December 2006 with Justice League of America #4 and #5.

Before that it was:
January 2006: Infinite Crisis #4 and All Star Superman #2
December 2005: Infinite Crisis #3 and All Star Batman and Robin the Boy Wonder #3
November 2005: Infinite Crisis #2 and All Star Superman #1
July 2004: Superman/Batman #11 and Superman #207
June 2004: Identity Crisis #1 and Superman #206
April 2004: Superman #204 and Superman/Batman #9
March 2004: Superman/Batman #8 and JLA/Avengers #4

Those are the only months since the final order era started in February 2003 when DC took the top two comics slots.
